Model Release and Casting Form
Welcome to our creative casting call! To be considered, you’ll need to complete this universal model release and application form below—it takes just a few minutes and includes consents for image use, deliverables, and shoot logistics. Castings fill up fast in NYC, so apply now; not all submissions can be selected, but we’re excited to review your vibe!
I consent to participating in this free casting session (~1-hour duration), during which photographs will be taken. I understand and agree that I will receive 10 professionally edited photos (selected from proofs with basic retouching). Video may also be captured if it fits the creative flow, but there is no guarantee of video footage or edited clips being produced or delivered. Any video clips provided (up to 2-3 short 10-30 second edits) will be at the photographer’s discretion. I acknowledge that all images and footage are for portfolio-building purposes and grant permission for their use in the photographer’s promotional materials, as outlined in the full model release.
I understand that additional edited photos or video clips beyond the free deliverables can be purchased if I am interested, with no obligation to buy. Pricing is $10 per extra edited photo and $15 per extra edited video clip (bundles available upon request). If I choose to purchase extras, I agree to pay upfront via [specify method, e.g., Zelle/PayPal] and receive them within 3-5 days of selection from proofs. This is optional and does not affect my participation in the free casting.

Content Usage Agreement (Required) I agree to the following terms for this free casting: For good and valuable consideration, I (if 18 or older) or my parent/legal guardian (if under 18) grant NEO Digital Imaging a non-exclusive, worldwide, royalty-free, perpetual, and irrevocable license to use, edit, reproduce, display, and distribute any photos or videos submitted or created during the session (the “Content”). This is limited to NEO’s website, official social media, and promotional materials (e.g., digital ads, email, portfolios, pitch decks) solely to promote NEO and its services—no further consent, approval, or compensation required. NEO will not sell, license to third parties, or use for sensitive/AI purposes without additional written consent. I represent I have legal authority to grant this.

Consent for Hair, Makeup, and Preparation I consent to arriving fully prepared for the free casting session (~1-hour duration), including handling my own hair, makeup, and styling to match the described looks or aesthetics in my application. I understand that no hair, makeup, or styling services will be provided on-site by the photographer or team. I will come camera-ready (e.g., natural or theme-appropriate makeup, styled hair) to ensure efficient use of time, and I acknowledge that changing facilities may be limited—public restrooms or nearby spots (e.g., in parks for outdoor shoots) can be used if needed, but it’s best to arrive shoot-ready to avoid delays.
