🆔 ID Verification (Required for Model Release) A valid photo ID is required to complete the model release. This ensures authenticity and legal verification for all participants. For Models Over 18: ✔️ A valid government-issued photo ID (driver’s license, passport, or state ID) For Models Under 18: ✔️ At least one parent’s or legal guardian’s valid photo ID (two encouraged) ✔️ Model’s ID (optional but encouraged) – such as a school ID or work permit Providing the necessary identification helps us maintain a secure and professional environment. Content Usage Agreement

I agree to grant NEO Digital Imaging the unrestricted right to use, edit, and distribute any photos or videos I submit, as well as any photos or videos taken during in-person sessions, for use on their website, social media, promotional materials, and other marketing or commercial purposes. I understand that this usage is perpetual, non-revocable, and does not require further consent or compensation.
